Founded in 2011, Innovative Integrated Health is a premier multi-ethnic integrative healthcare provider focused on superior care management for elderly persons residing in the Central Valley counties of Fresno, Kern and Tulare, as well as in Orange County. Innovative Integrated Health provides elderly individuals with health care services in the privacy of their homes through its Program of All-Inclusive Care for the Elderly (PACE) — a unique healthcare program that offers a multitude of services to seniors who have chronic illness or disabilities.
It is our mission to provide our participants a comprehensive and integrated health care offering encompassing a variety of services such as specialty and primary doctor care, emergency and acute hospital care, audiology, optical, prescription, durable medical equipment, nursing care and physical therapy. A little prevention goes a long way when it comes to our members. Aside from our extensive medical benefits of being an IIH PACE member, your loved ones have access to social services, counseling, nutrition services, chaplain/spiritual care and recreational exercise in order to make their lives easier. Becoming a IIH PACE member means that your senior loved ones can participate and enjoy wholesome, stimulating daily activities that will help improve and maintain their mental and social health. In our effort to provide full support for your loved one, we understand that getting around can often be a challenge. Transportation services are provided upon request. Our home care Personal Care Assistants provide a range of essential tasks from records maintenance, personal care, medication schedules and respiratory care. Other services include light house cleaning, meal preparation, laundry and shopping. Our staff is composed of trained professionals as well as loving caregivers who put the total health and well-being of each member first. Innovative Integrated Health’s PACE program accepts Medicare and/or Medi-Cal and its members have no out-of-pocket costs for healthcare services, prescriptions or any of our services.
